Join Our Practice

Before scheduling your first appointment, there are a few steps you need to take to join the practice.

1. Check Your Insurance Coverage

Insurance coverage is required for new members. We accept most major insurance plans, including Blue Cross/Blue Shield, Tufts, Harvard Pilgrim, Medicare, Aetna, United Health Care, Cigna, Fallon, and Allways. 

We do not accept lower-tier health plans that major insurers offer such as Tufts Direct plans, Commonwealth Care Alliance, Senior Whole Health, Wellsense, Medicare Advantage Stride HMO, or Mass Health. Please call the office to ensure we accept your plan. Should you change plans after establishing care with us please check first to ensure we take your new health plan or plan to transition your care to another office.

2. Download and Return New Patient Forms

Forms need to be filled out completely prior to joining our practice. This ensures we understand your whole health picture, medical history, and current conditions, as well as helps inform us if we’re a good fit for your care.

Return your complete forms to our office by scanning/emailing to lfrodema@phopkinsmd.com; faxing them to 617-769-9952; or mailing them to 571 Main St, Weymouth, MA, 02190.

3. Wait For a First Consultation Call

Once you’ve returned your completed New Patient Forms, our intake nurse will call to review your answers and determine if Hopkins Medical Group is a good fit for your care. Upon approval, you will schedule your first appointment on that call.

4. Become a GenWell Member Prior to Your First Appointment

At any point before your first appointment, finish joining our practice by signing up for a GenWell membership. This gives you access to our practice’s primary care services, specialty referrals, telemedicine opportunities, educational resources, and more.
